Technical Article Abstract
Inhalation has long been seen as a route of delivery
for drugs; in 1860, Henry Hyde Salter published a
systematic review of treatment options distinguishing
depressants (e.g. tartar-emetic), stimulants (e.g.
coffee) and sedatives (e.g. opium). However,
despite the prevalence of inhaled medications today
for topical treatment of the respiratory tract, there
have been few products launched in recent years
aimed at the systemic delivery of drugs via the
lungs. This is not through a lack of interest; there
are a large number of potential applications in the
pipeline. Yet, of almost 100 new products, only a
few per cent are using existing delivery systems, so
that the developers face the double challenge of
developing a new indication/route of administration
and a new device.

The first pressurised metered dose inhaler (pMDI)
was launched by Riker Laboratories in 1956. Prior to
this asthma sufferers had to manage their therapy
using glass nebulisers, which were awkward and
often broken. One day, whilst struggling with her
squeeze bulb nebuliser, the daughter of the president
of Riker Laboratories, Inc., asked her father “why
can’t they put my asthma medicine in a spray-can
like they do hair spray?”.1 George Maison took this
question seriously and put the idea forward to Irvine
Porush and his team in Riker’s pharmaceutical
development laboratory.
